What Is Hormone Replacement Therapy?
Hormone Replacement Therapy (HRT) is a medical treatment designed to restore hormone levels that naturally decline during perimenopause and menopause.
As estrogen, progesterone, and other key hormones fluctuate, many women experience symptoms such as:
- hot flashes and night sweats
- mood swings or anxiety
- fatigue and brain fog
- weight changes
- sleep disruption
- reduced libido
HRT works by safely replenishing hormones to support the body’s natural balance, helping relieve symptoms and improve overall wellbeing.

Conversations with Dr. Whitney on HRT
Dr. Whitney Elsesser, DNP, CNM
Certified Nurse Midwife & Wellness Provider
Dr. Whitney Elsesser has spent over a decade in women’s health. As a certified nurse midwife and wellness provider at About Face, she sits across from women every day who have been told their labs look fine — yet still don’t feel like themselves. We asked her to set the record straight on the misconceptions that keep women from getting the care they actually need.
What are the most common misconceptions women come in with about hormone therapy?
Two come up constantly. The first is testosterone. A lot of women don’t even realize testosterone is something they should be thinking about — it’s not just a men’s hormone. Women need it too, and when levels are optimized, the difference in energy, motivation, and mental clarity can be significant. The misconception that it will cause unwanted physical changes keeps a lot of women from exploring something that could genuinely help them.
The second is progesterone — specifically, who needs it. Most people associate progesterone exclusively with reproductive health, but it’s so much more than that. Progesterone is a calming, sleep-supporting hormone. Women who struggle with PMS, poor sleep, irritability, or emotional dysregulation often have a progesterone imbalance. It’s one of the most commonly overlooked pieces of the puzzle in conventional care, and for many women, addressing it makes a meaningful difference.
A lot of women are told their hormone levels are fine but they still don't feel their best. How do you address that?
This is the question I get more than any other. Women come in and say their doctor told them everything looks great — and they still don’t feel good.
Here’s what’s happening: conventional medicine evaluates hormone levels against a broad baseline. During perimenopause and menopause, declining hormone levels are expected. But “expected” doesn’t mean they aren’t causing real symptoms — or that you should just live with them.
The analogy I always use: it’s entirely normal to fall and break your leg. But we don’t leave it broken just because the injury makes sense. We fix it. Your hormones work the same way.
When they decline and you’re symptomatic, the right move is to restore them and work toward getting you feeling like yourself again.
There’s a meaningful difference between a baseline reading and an optimized one. Our goal is always the latter.
Why has hormone health been so misunderstood for so long?
A lot of it traces back to one of the most well-known research studies on women’s hormones, which did not isolate the outcomes of synthetic versus bioidentical hormones. When those results shaped the broader conversation around HRT, the nuance got lost — and the resulting narrative did a disservice to women who could have benefited from a more complete understanding of their options.
Bioidentical hormones are designed to work with your body. They’re delivered topically — as a cream or a patch — or through a pellet placed just under the skin, absorbing steadily over time without the peaks and valleys of other methods. The pellet, in particular, is something I’m a big advocate for. It goes just beneath the skin and provides a consistent, steady release that many patients find makes a real difference in how they feel day to day.
The bottom line: bioidentical hormone therapy is well-tolerated, effective for many patients, and something far more women should know is available to them.
What do women commonly experience on hormone therapy?
It looks different for every patient, and we always set expectations accordingly. But there are patterns worth knowing about.
With progesterone, many women report sleeping better and feeling less reactive. When progesterone is low, everyday stress can feel harder to manage — the frustration, the mood swings, the sense of not handling things the way you used to. Restoring balance can shift that for a lot of women.
With estrogen, the range of benefits can be surprising. Many women see improvement in hot flashes, joint comfort, and skin quality — estrogen is a precursor to collagen, so its effects show up in more places than people expect. Pelvic floor issues that women have quietly accepted are often worth discussing with a provider, because there may be more options than they realize. I had a patient recently who gave me a hug and said, “I didn’t know I could have painless sex.” She had just accepted that this was her life. It was a good reminder of why this conversation matters.
